In consideration of being permitted to participate in the event specified below (“Event”), I hereby affirm and agree as follows:

1. Voluntary Participation

I am freely and voluntarily choosing to participate in the Event after carefully and fully reading, affirming, and agreeing to all of the terms of this Release, Waiver, and Indemnity Agreement (the “Agreement”).

2. Assumption of All Risks

I hereby assume any and all of the risks of participating in the Event including but not limited to, any risks that may arise from the negligent acts or omissions and/or from dangerous or defective equipment or property owned, maintained, operated or controlled by of the Entrepreneurs’ Organization (“EO”), all EO Chapters, their officers, board members, employees, agents, representatives, volunteers, affiliates, partners, joint ventures, contractors, vendors, sponsors, and their successors and assigns (collectively the “Host”). ; I understand that all activities of the Event are subject to change at the sole discretion of the Host without notice. I also understand that incidental to my participation in the Event, I may engage in or be subject to activities that involve the risk of serious personal injury, illness, permanent disability, dismemberment, and death, and that my participation in the Event may also involve the risk of severe economic and property loss, theft, and other damages. I understand that these risks may result from the actions, omissions, negligence, and failure to act of myself and others and from the condition of any property, facilities or equipment used during the Event. I also understand that there may be risks involved which are not known, foreseen, or reasonably foreseeable to me or the Host at this time or at the time of the Event. With full knowledge of all of the foregoing risks (the “Assumed Risks”), I hereby affirm that I am electing to voluntarily participate in the Event and agree to assume all of the Assumed Risks arising out of or relating to my participation in the Event, including without limitation those enumerated above, and accept personal responsibility for any injury of any kind or nature that I or my property may suffer arising out of or in connection with my participation in the Event.

3. Condition and Medical Authorization.

I hereby affirm, represent, and certify that I am physically and mentally fit for participation in the Event. In connection with any injury sustained, illness, or medical condition experienced during my participation in the Event, I authorize any emergency first aid, medication, medical treatment, or surgery (collectively “Care”) deemed necessary by the attending medical personnel if I am not able to choose on my own behalf. Additionally, I authorize medical treatment for me, at my cost, if the need arises; however, I acknowledge that the Host shall have no duty, obligation or liability arising out of the provision of, or failure to provide, Care.

4. Release, Waiver and Indemnity

For myself, my guests, invitees, executors, administrators, heirs, successors and assigns, I hereby fully waive and release the Host of and from any and all claims, demands, causes of action, and liabilities, including but not limited to, those arising from the Assumed Risks. I further agree to indemnify, defend, and hold harmless the Host from and against any and all claims, demands, causes of action, and liabilities arising out of or relating to my participation in the Event. This Agreement shall be construed in accordance with the laws of India.

5. Photo/Video Consent and Release

I hereby grant Host permission to take and use photographs, videos and other images or depictions (collectively “Images”) of me in any publications, releases, materials, or media, including but not limited to print and digital media, websites, or other electronic communications. I agree that my name and identity may be revealed in descriptive text or commentary in connection with the Images, and I authorize the use of the Images without compensation to me. All negatives, prints, digital reproductions shall be the property of the Host.

6. I hereby declare that I have been duly informed about the zero tolerance policy in regard to the use of illicit drugs, alcohol and any other contravened substances, which is not permitted under the laws of India. I further declare that it shall be my personal responsibility and neither organiser nor any of the participants shall be held responsible for that.

7. I expressly acknowledge and agree that the foregoing Release, Waiver, and Indemnity Agreement is intended to be as broad and inclusive as is permitted by laws of India and that if any portion thereof is held invalid, it is agreed that the balance shall; notwithstanding, continue in full legal force and effect.

8. I HAVE READ, FULLY UNDERSTAND, AND VOLUNTARILY AGREE THIS RELEASE, WAIVER, AND INDEMNITY AGREEMENT, and attest and affirm that no oral or written representations, statements, or inducements apart from this Agreement have been made to me.
